Postgraduate Specialization Eligibility:

MCPS (Ophthalmology)	MBBS Degree from PMDC recognized institution. Minimum CGPA 3.50 Valid Registration with Pakistan Medical and Dental Council. Completed one year house job in PMDC recognized institution. Foreign Graduates will have to produce equivalence of their degree.	Five passport size photographs with white background. MBBS degree from PMDC recognized institution. MBBS consolidated marks certificate/transcript.  Valid PMDC Registration Certificate. o House Job Certificates (One Year) Domicile Certificate Valid Computerized National Identity Card (CNIC) N.O.C for In-service Candidates from their employer (Mandatory)
FCPS-II (Ophthalmology)	MBBS Degree from PMDC recognized institution. Valid Registration with Pakistan Medical and Dental Council. Completed one year house job in PMDC recognized institution. Candidates who have passed the FCPS-I in Ophthalmology are eligible to apply for the FCPS-II ophthalmology residency program within a three-year period. Foreign Graduates will have to produce equivalence of their degree.	Five passport size photographs with white background MBBS degree from PMDC recognized institution. MBBS consolidated marks certificate/transcript. FCPS-I in Ophthalmology passed / Congratulation Letter. Valid PMDC Registration Certificate. House Job Certificates (One Year) Domicile Certificate
2ND Fellowship in Orbit and Oculoplastic	MBBS Degree from PMDC recognized institution. Candidate who has passed FCPS-II in Ophthalmology is eligible to apply. The qualification of FCPS-II Ophthalmology must be registered with Pakistan Medical and Dental Council. Foreign Graduates will have to produce equivalence of their degree.	Five passport size photographs MBBS degree from PMDC recognized institution. MBBS consolidated marks certificate/transcript. How to Register

Application Process:

• Please fill in all the parts of application form in your own hand writing. Incomplete form will not be entertained
• Please write in Capital and use black ink.
• Photocopies of all relevant documents shall be attested by an officer of BPS-18 or above.
• In-service candidates will have to obtain no objection certificate from their employer for applying in Postgraduate courses, however they can send advance copy along with relevant documents.
• Bank Draft of Rs.5000/= in favor of Sindh Institute of Ophthalmology and Visual Sciences (SIOVS) Hyderabad.
• Completed application form with documents shall be submitted at Directorate of Academics, SIOVS Hyderabad.
• Application Form & Prospectus Charges: Rupees. 5000/-.
