Lady Health Visitor Course

Matric with Science (Physics, Chemistry and Biology as compulsory subjects) FSC (Pre-Medical) with minimum 50% marks from any recognized Board. Applying candidate should have ID Card/ B Form. Applying candidate should have domicile of Punjab. Hafiz-e-Quran Candidate should have Certificate from Registered Institution. Medical Fitness Certificate from any Medical Officer. Maximum age limit will be 35 years which shall be calculated till last date of application. (No age relaxation will be given beyond the permitted age limit of 35 years in any case under any circumstances).

Merit Determination Criteria:

• Merit will be determined on the basis of marks of FSC. (Premedical).
• In case two or more candidates have equal marks in FSC (Premedical) then the merit shall be determined on the basis of obtained marks in Matric with Science.
• In case two or more candidates have equal marks in FSC (Pre-medical) as well as in Matric (with Science) then merit will be determined on the basis of seniority with respect of upper age of the candidate.
• In case of all marks/parameters given above are equal then case will be decided by the SHCME Department.

How to Register

Admission Guidelines:

• Admissions will be made purely on merit basis as per eligibility criteria according to the Government Policy through centralized/online system.
• Necessary instructions/ guidelines, schedule and merit list etc. will be uploaded from time to time on the web-portal to facilitate the candidates.
• Before applying through online system, candidates must read Instructions and Policy available on the web-portal.
• The candidates shall have to get themselves registered on the web-portal http://nursing.phf.gop.pk  to apply through centralized online system.
• The candidates shall upload all the requisite original/ attested documents i.e. copy of CNIC (Front & Back side), Domicile, Matric Certificate and FSC Certificate.
• The admission shall only be confirmed after verification of credentials/educational documents including IBCC. In case of any wrong information/fake/fabricated documents found at any stage, admission shall be cancelled along with disciplinary proceedings.
• The candidates shall have to pay an amount of Rs.300/- (Rupees Three Hundred only) as processing fee which is non-refundable.
• Fee submission shall only be made on the voucher generated/downloaded from the portal.
• The fee shall be deposited in the designated account of the PHF in any branch of Bank of Punjab well before the deadline of application submission to avoid any technical issue.
• The receipt shall be uploaded with the online application on http://nursing.phf.gop.pk 
  
• Deadline for submission of application is 04:00pm PST on 08th February, 2025, In case of public holiday on closing date, the deadline shall be 04:00 pm PST on next working day.
• Applications after deadline shall not be entertained.
• Application received by mail directly to Specialized Healthcare & Medical Education Department/ Punjab Health Foundation or Directorate General Nursing, Punjab, Lahore or respective Nursing College SHALL NOT be considered/entertained.
• Only completely submitted ONLINE applications with all documents and application fees on the PHF Portal shall be entertained.
  
• If a candidate, after her selection on merit, fails to give online consent within the stipulated time period as mentioned on the portal, her name will be struck off from the admission process and the resultant vacant seat will be offered to the candidate who falls next on merit.
• Selected Students shall be admissible for stipend according to Government Policy.
• Hostel facility shall be given to the selected candidates.
